Guide to application for qualifications recognition of safety production testing and inspection institutions

This guide provides essential information to institutions in Hainan on applying for qualifications recognition as a safety production testing and inspection institution.

Basic requirements
The applicant institution shall satisfy the conditions for qualifications recognition as a safety production testing and inspection institution as stipulated in the Measures for the Administration of Safety Evaluation, Testing and Inspection Institutions, including:

1. The institution shall be an independent legal entity with fixed assets of no less than 10 million yuan ($1.49 million).

2. The institution shall have premises with a gross floor area of no less than 1,000 square meters and possess facilities, equipment and environmental conditions suitable for conducting safety production testing and inspection activities. The original value of its testing and inspection facilities and equipment shall be no less than 8 million yuan.

3. An institution engaging in a single business sector shall employ no fewer than 25 professional and technical personnel. For each additional industry sector, at least five additional professional and technical personnel shall be employed. Among all professional and technical personnel:
(1) Registered safety engineers with intermediate or higher qualifications shall account for no less than 30 percent;
(2) Personnel holding intermediate or higher professional titles shall account for no less than 50 percent;
(3) Personnel holding senior professional titles shall account for no less than 25 percent.

4. Professional and technical personnel shall possess the professional skills required for safety production testing and inspection activities and shall have at least two years of relevant industry experience.

5. The legal representative shall provide a commitment letter acknowledging and undertaking the legal responsibilities, obligations, rights and risks associated with safety production testing and inspection activities.

6. The person responsible for safety production testing and inspection activities, the technical director and the quality director shall hold senior professional titles and have at least eight years of relevant industry experience.

7. The institution shall establish a documented management system that complies with the General Requirements for the Competence of Safety Production Testing and Inspection Institutions and other applicable standards and normative documents.

8. The institution shall maintain a functioning website through which institutional information is publicly accessible.

9. The institution shall have no record of major illegal conduct or serious dishonesty within the three years preceding the application date.

10. Other conditions stipulated by applicable laws, administrative regulations and relevant standards shall be satisfied.

Required materials
1. Catalogue of application materials (including the checklist of application materials);
2. Application form for qualification recognition of a safety production testing and inspection institution;
3. Certificate of legal entity status;
4. Supporting documents showing and evidencing the building area of the workplace;
5. Supporting documents showing and evidencing fixed assets, or a written commitment;
6. Supporting documents showing and evidencing the original value of testing and inspection facilities and equipment;
7. Written commitment confirming that the number, educational qualifications and professional titles of professional and technical personnel meet the applicable requirements;
8. Written commitment confirming that the work experience of the person responsible for safety production testing and inspection activities, the technical director and the quality director meets the applicable requirements;
9. Certificate of inquiry results or declaration confirming no record of major illegal conduct or serious dishonesty within the three years preceding the application date;
10. Commitment letter of the legal representative;
11. Management system documents (non-controlled version).
